When solving problems, like making predictions or choices, people often “sample” possibilities into mind. Here, we consider whether there is structure to the kinds of thoughts people sample by default—that is, without an explicit goal. Across three experiments we found that what comes to mind by default are samples from a probability distribution that combines what people think is likely and what they think is good. Experiment 1 found that the first quantities that come to mind for everyday behaviors and (...) events are quantities that combine what is average and ideal. Experiment 2 found, in a manipulated context, that the distribution of numbers that come to mind resemble the mathematical product of the presented statistical distribution and a (softmax-transformed) prescriptive distribution. Experiment 3 replicated these findings in a visual domain. These results provide insight into the process generating people’s conscious thoughts and invite new questions about the value of thinking about things that are both likely and good. (shrink)

Mission-oriented climate change research is often unverifiable. Therefore, many stakeholders look to peer-reviewed climate change research for trustworthy information about deeply uncertain and impactful phenomena. This is because peer-review signals that research has been vetted for scientific standards like reproducibility and replicability. Here we evaluate the transparency of research methodologies in mission-oriented computational climate research. We find that only five percent of our sample meets the minimal standard of fully open data and code required for reproducibility and replicability. The widespread (...) lack of minimal openness standards in peer-reviewed climate change research jeopardizes scientific credibility and risks poor societal outcomes as we manage growing climate risks. The book starts out by presenting a theoretical framework on eHealth and surveillance, followed by an introduction to the various ideas on eHealth and surveillance explored in the subsequent chapters. Issues addressed in the chapters include privacy and data protection, social acceptance of eHealth, cost-effective and innovative healthcare, as well as the privacy aspects of employee wellness programs using (...) eHealth, the use of mobile health app data by insurance companies, advertising industry and law enforcement, and the ethics of Big Data use in healthcare. A closing chapter draws on the previous content to explore the notion that people are 'under observation', bringing together two hitherto unrelated streams of scholarship interested in observation: eHealth and surveillance studies. Self-reported data are regarded by medical researchers as invalid and less reliable than data produced by experts in clinical settings, yet individuals can increasingly contribute personal information to medical research through a variety of online platforms. In this article we examine this ‘participatory turn’ in healthcare research, which claims to challenge conventional delineations of what is valid and reliable for medical practice, by using aggregated self-reported experiences from patients and ‘pre-patients’ via the internet. We focus on 23andMe, a genetic testing (...) company that collects genetic material and self-reported information about disease from its customers. Integral to this research method are relations of trust embedded in the information exchange: trust in customers’ data; trust between researchers/company and research subjects; trust in genetics; trust in the machine. We examine the performative dimension of these trust relations, drawing on Shapin and Schaffer’s discussion of how material, literary and social technologies are used in research in order to establish trust. Our scepticism of the company’s motives for building trust with the self-reporting consumer forces us to consider our own motives. How does the use of customer data for research purposes by 23andMe differ from the research practices of social scientists, especially those who also study digital traces? By interrogating the use of self-reported data in the genetic testing context, we examine our ethical responsibilities in studying the digital selves of others using internet methods. How researchers trust data, how participants trust researchers, and how technologies are trusted are all important considerations in studying the social life of digital data. (shrink)

Recent medical informatics and sociological literature has painted the image of a new type of patient—one that is reflexive and informed, with highly specified information needs and perceptions, as well as highly developed skills and tactics for acquiring information. Patients have been re-named “reflexive consumers.” At the same time, literature about the questionable reliability of web-based information has suggested the need to create both user tools that have pre-selected information and special guidelines for individuals to use to check the individual (...) characteristics of the information they encounter. In this article, we examine suggestions that individuals must be assisted in developing skills for “reflexive consumerism” and what these particular skills should be. Using two types of data (discursive data from websites and promotional items, and supplementary data from interviews and ethnographic observations carried out with those working to sustain these initiatives), we examine how users are directly addressed and discussed. We argue that these initiatives prescribe skills and practices that extend beyond finding and assessing information on the internet and demonstrate that they include ideals of consumerism and citizenship. Various medical, political, and independent organizations have created user-friendly tools for finding reliable medical/health information on the web and have been faced with the challenge of defining what it means for information to be reliable. Little attention has been given to the work of reviewing web-based information and applying selection criteria to individual sites. In this article, (...) the authors examine how guidelines are applied in practice and discuss how the selection criteria and the practices of applying them reinforce distinctions that current medical sociology and informatics literature suggests have been broken down by internet technologies and the availability of web-based medical information. Zoos are typically public attractions that do not explicitly, or through a more implicit culture, expect quietness from their guests. This paper will explore whether quietness is something we should aim for when we are visiting zoos. Primarily through analogy with other public spaces which share some of the key characteristics of zoos (libraries and schools, cinemas, theatres and galleries, war memorials, and hospitals and gardens), we suggest that quiet is indeed appropriate in zoos (more appropriate than being noisy). A (...) major component of this argument will be the exploration of what is meant by quiet (and noise), and outlining a concept of quietness based on an idea of attention. The central premise here, drawing upon theories of attention and love, is that noise involves a certain kind of outward expression, which leaves less room for the appreciation of, and attention to, the animals and information that the zoo provides. The article will reflect on how a call for quiet may create scope for enhancing the educational possibilities of zoos, and how zoos might profit by taking the acoustic dimension of visitor behaviour into account when considering their values. Abstract25.6 Million people in the United States have Limited English Proficiency (LEP), defined as insufficient ability to read, write, or understand English. We will (1) Delineate the merits of approaching language as a social determinant of health, (2) highlight pertinent public health values and guidelines which are most relevant to the plight of populations with LEP and (3) Use the COVID-19 pandemic as an example of how a breakdown in public health ethics values created harm for populations and patients with (...) LEP. We define a framework to tease out public health responsibilities given some populations’ limited proficiency in a society’s predominant language. The American Public Health Association (APHA) public health ethics core values serve as a framework to interrogate current practices. We use the COVID-19 case to illustrate gaps between health policy and healthcare disparities experienced by populations with LEP. (shrink)

Whole–exome sequencing (WES) has increased our ability to analyze large parts of the human genome, bringing with it a plethora of ethical, legal, and social implications. A topic dominating discussion of WES is identification of “secondary findings” (SFs), defined as the identification of risk in an asymptomatic individual unrelated to the indication for the test. SFs can have considerable psychosocial impact on patients and families, and patients with an SF may have concerns regarding genomic privacy and genetic discrimination. The Genetic (...) Information Nondiscrimination Act of 2008 (GINA) currently excludes protections for members of the military. This may cause concern in military members and families regarding genetic discrimination when considering genetic testing. In this report, we discuss a case involving a patient and family in which a secondary finding was discovered by WES. The family members have careers in the U.S. military, and a risk–predisposing condition could negatively affect employment. While beneficial medical management changes were made, the information placed exceptional stress on the family, who were forced to navigate career–sensitive “extra–medical” issues, to consider the impacts of uncovering risk–predisposition, and to manage the privacy of their genetic information. We highlight how information obtained from WES may collide with these issues and emphasize the importance of genetic counseling for anyone undergoing WES. (shrink)

The article presents information on the capabilities of Dretske-style analysis of knowing (DAK) and of several competing analyses with respect to accounting for the apparent facts. It informs that the DAK can ground plausible verdicts about knowledge and ignorance in cases involving lotteries. It further informs that the knowledge-efficacy donor imply the implausible thesis.

Background: Medical tourism involves patients travelling internationally to receive medical services. This practice raises a range of ethical issues, including potential harms to the patient's home and destination country and risks to the patient's own health. Medical tourists often engage the services of a facilitator who may book travel and accommodation and link the patient with a hospital abroad. Facilitators have the potential to exacerbate or mitigate the ethical concerns associated with medical tourism, but their roles are poorly understood. -/- (...) Methods: 12 facilitators were interviewed from 10 Canadian medical tourism companies. -/- Results: Three themes were identified: facilitators' roles towards the patient, health system and medical tourism industry. Facilitators' roles towards the patient were typically described in terms of advocacy and the provision of information, but limited by facilitators' legal liability. Facilitators felt they played a positive role in the lives of their patients and the Canadian health system and served as catalysts for reform, although they noted an adversarial relationship with some Canadian physicians. Many facilitators described personally visiting medical tourism sites and forming personal relationships with surgeons abroad, but noted the need for greater regulation of their industry. -/- Conclusion: Facilitators play a substantial and evolving role in the practice of medical tourism and may be entering a period of professionalisation. Because of the key role of facilitators in determining the effects of medical tourism on patients and public health, this paper recommends a planned conversation between medical tourism stakeholders to define and shape facilitators' roles. A Theory of Virtue is a systematic, comprehensive framework for thinking about the moral evaluation of character, proposing that virtue is chiefly a matter of being for what is good, and that virtues must be intrinsically excellent and not just beneficial or useful.

From nineteenth-century abolitionism to Black Lives Matter today, progressive social movements have been at the forefront of social change. This book is meant to be a primer, that is, an introduction, to probability logic, a subject that appears to be in its infancy. Probability logic is a subject envisioned by Hans Reichenbach and largely created by Adams. It treats conditionals as bearers of conditional probabilities and discusses an appropriate sense of validity for arguments such conditionals, as well as ordinary statements as premisses. This is a clear well-written text on the subject of probability logic, suitable for advanced undergraduates (...) or graduates, but also of interest to professional philosophers. There are well-thought-out exercises, and a number of advanced topics treated in appendices, while some are brought up in exercises and some are alluded to only in footnotes. By this means, it is hoped that the reader will at least be made aware of most of the important ramifications of the subject and its tie-ins with current research, and will have some indications concerning recent and relevant literature. There is little information regarding arterial elasticity in heart transplant recipients. This study aimed to characterize small and large artery elasticity in (...) heart transplant recipients in comparison with an asymptomatic population free of overt cardiovascular disease. A second goal was to identify demographic and clinical factors associated with arterial elasticity in this unique population.Methods: Arterial pulse waveform was registered noninvasively at the radial artery in 71 heart transplant recipients between 2008 and 2010. SAEs and LAEs were derived from diastolic pulse contour analysis. Comparisons were made to a healthy cohort of 1,808 participants selected from our prevention clinic database. Multiple regression analyses were performed to evaluate associations between risk factors and SAE and LAE within the heart transplant recipients.Results: LAE and SAE were significantly lower in heart transplant recipients than in the normal cohort. Female sex and history of ischemic cardiomyopathy were significantly associated with reduced LAE and SAE. Older age and the presence of moderate cardiac allograft vasculopathy were also significantly associated with reduced SAE. Transplant duration was associated with increased SAE.Conclusion: Heart transplants are associated with peripheral endothelial dysfunction and arterial stiffness, as demonstrated by a significant reduction in SAE and LAE when compared with a healthy population. Furthermore, cardiac allograft vasculopathy is associated with a significant reduction in SAE, suggesting a potential use in screening heart transplant recipients at risk for developing this complication.Keywords: heart transplant, endothelial function, arterial elasticity, cardiac allograft vasculopathy. The time has long passed when it was necessary to explain or to justify philosophy of science to a convention of scientists. The growing intent among philosophers and scientists to clarify and perhaps to unify the programs, methods, and results of the disciplines of philosophy and science is matched by a growing determination among the special sciences to cooperate in the examination of fundamental concepts and presuppositions in the light of the positive results of science, to cooperate in the systematic (...) doubting of these positive results, and to cooperate in the thoroughgoing analysis and critique of logic and of language,—all of which are projects which can be carried out only by the joint effort of philosophers and scientists. In short, it has long since been realized that the special sciences cannot safely be abandoned to their devotees, exclusively. It was only twenty-five years ago that Santayana was able to say “I would gladly learn more of contemporary thinkers if they would only learn more of each other.” The implication that lies in that statement is hardly the case today, when the synoptic enterprise of philosophy-of-science is helping the scientific thinker to recognize the limitations of his task and the assumptions with which he starts, and to save him from being too easygoing in the criticism of his categories. The SERVE Program at Ignatius University seeks to foster the ethical development of its participants by combining academic study of philosophy and theology with a year-long community service project. This study considered the impact of the SERVE Program upon Ignatius University students majoring in business in comparison to students pursuing majors in the liberal arts, education, and nursing. Findings from this study offer insight into the response of business students to ethical content in comparison to students pursuing degrees in other (...) disciplines. We first argue that the question has no easy answer in either of the two approaches. We then draw a parallel between functions and goal directedness. We conclude by proposing an answer to the question: The explanatory importance of nonsupervenient (...) properties (like having the function of doing something, or like being goal-directed) does not lie in any special causal mechanism through which these properties bring about their effects; it lies rather in the different classification of the explananda types that these properties generate. In this chapter, I mount an ad hominem argument that it is unreasonable for people to strike the latter life posture apart from belief in God. I review nontheistic alternatives – life postures that qualify realism, dampen hopes, curtail on meaning‐making, or aim at getting beyond personality altogether. I defend affirmative action on the ground that it increases certain people’s ability to exercise their basic liberties, rather than because it rectifies injustice in the narrow context of educational admission procedures. I present this justification using a Rawlsian contractualist framework to forge a “nonideal principle of justice.” Drawing on social science, I argue that this principle supports affirmative-action policies like those in the contemporary U.S., and blocks the objection that such policies are unfair. In closing, I show how my (...) account can be used to refine some features of contemporary affirmative-action policies, and I reflect more generally on the value of nonideal principles of justice for tackling exigent topics. The relation between government and industry is a problem not of economics but of political economy. It is a problem of controlling and civilizing economic power in order that it may be used for the public good within the framework of democratic institutions. It is a problem of striking a delicate balance between individual freedom and centralized control. It is a problem for which there is no monistic solution and which transcends the clichés of popular controversy between conservatives and liberals.

For at least two millennia, religious traditions, spiritual communities and secular moral thinkers have debated the nature and sources of forgiveness. But near the end of the twentieth century understanding forgiveness took on new urgency, as divided societies looked to forgiveness as a vehicle of reconciliation, governments sought forgiveness for past wrongs, and popular psychology explored the therapeutic effects of forgiveness. These developments have led to a remarkable increase in scholarship on forgiveness: philosophers examine its moral nature; psychologists seek to (...) measure it and determine its contribution to well-being; historians and political theorists consider its relevance to national projects of reconciliation.Earlier versions of this paper were delivered to audiences at Syracuse University and Seton Hall University. Schleiermacher’s theology of absolute dependence implies that absolutely everything, including evil, including even sin, is grounded in the divine causality. In addition to God’s general, creative causality, however, he thinks that Christian consciousness reveals a special, teleologically ordered divine causality which is at work in redemption but not in evil. He identifies good and evil, respectively, with what furthers and what obstructs the development of the religious consciousness in human beings. In the wake of the digital, some have recommended that we abandon the tedium of teaching handwriting to children in service of promoting “more creative” digital literacies. Others worry that an early diet of keyboard and screen may have deleterious effects on children's social, emotional, and cognitive development, as well as their physical well-being. Yet in this debate, the algorithmic scripts and digital surfaces underwriting these new reading, writing, and mathematical practices are, with a few notable exceptions, almost exclusively ignored. In this essay, Catherine Adams asks whether the digital, and the reading and writing spaces it affords, are of consequence to our habits of thinking and ways of being, particularly in light of the possible obsolescence of pen and paper in schools. Background: The field of clinical ethics is examining ways of determining competency. The Assessing Clinical Ethics Skills (ACES) tool offers a new approach that identifies a range of skills necessary in the conduct of clinical ethics consultation and provides a consistent framework for evaluating these skills. Through a training website, users learn to apply the ACES tool to clinical ethics consultants (CECs) in simulated ethics consultation videos. The aim is to recognize competent and incompetent clinical ethics consultation skills by watching (...) and evaluating a videotaped CEC performance. We report how we set a criterion cut score (i.e., minimally acceptable score) for judging the ability of users of the ACES tool to evaluate simulated CEC performances.Methods: A modified Angoff standard-setting procedure was used to establish the cut score for an end-of-life case included on the ACES training website. The standard-setting committee viewed the Futility Case and estimated the probability that a minimally competent CEC would correctly answer each item on the ACES tool. The committee further adjusted these estimates by reviewing data from 31 pilot users of the Futility Case before determining the cut score.Results: Averaging over all 31 items, the proposed proportion correct score for minimal competency was 80%, corresponding to a cut score that is between 24 and 25 points out of 31 possible points. The standard-setting committee subsequently set the minimal competency cut score to 24 points.Conclusions: The cut score for the ACES tool identifies the number of correct responses a user of the ACES tool training website must attain to “pass” and reach minimal competency in recognizing competent and incompetent skills of the CECs in the simulated ethics consultation videos. The application of the cut score to live training of CECs and other areas of practice requires further investigation. (shrink)

The sense of moral horror at certain deeds and the related idea of the sacred have not been given as central a place in ethical theory, theological or secular, as they have in our moral consciousness. I place them in a broader theological metaethics, in a way that I hope avoids mere taboo and provides for a rational critique of our responses. Moral horror is understood here in terms of violation of the sacred, and the sacred is understood in terms (...) of images of God. The focus on images of God is defended against a less ontological approach suggested by Ronald Dworkin's recent discussion of the sacred, and the choice of violation rather than defilement as a central concept is defended in dialogue with Jeffrey Stout's discussion of abominations. Questions have been raised over the acceptability of conducting human challenge studies in low and middle income countries. Most of these concerns are based on theoretical considerations and there exists little data on the attitudes of stakeholders in these countries. This study examines the view of researchers and REC members in Thailand regarding the design and conduct of challenge studies in the country. A questionnaire was developed based on ethical frameworks for human challenge studies. The target respondents included those who (...) had experience with health-related research at universities, non-university hospitals, and research institutes. A total of 240 respondents completed the on-line survey. In general, the respondents felt that the ethical issues raised by human challenge studies in LMICS do not differ significantly from those in high income countries, including: scientific rationale, safety, appropriate risks, and robust informed consent process. In contrast, issues that have been described as important for human challenge studies in LMICs were rated as having lower importance, including: a publicly available rationale, national priority, and community engagement.
